Who should complete this form? 

Please complete this form if you are purchasing an ETR program for use as a:

• Homeschool educator or parent educator

• Homeschool cooperative

• Independent educator or tutor

• Microschool or learning pod

• Private educational program

• Similar individual or small-group educational setting

What information will I need?

The form will ask for basic information about you and your educational setting, including:

• Your name and contact information
• The type of educational setting
• The estimated number of students who will use the program
• One reasonable form of information demonstrating educational use

You do not need to be a licensed classroom teacher to submit the form.

What can I use for verification?

You may provide one of the following:

• Homeschool registration, declaration, or acknowledgment, if applicable in your state
• Membership in a homeschool association or cooperative
• A website or public listing for your educational program
• An educator credential or professional certification
• Business documentation for a tutoring or educational program
• Documentation from an educational organization
• Another form of information that reasonably demonstrates educational use

If none of these options are available, you may explain your educational setting in the form. ETR will review the information and contact you if anything else is needed.

What happens after I submit the form?

ETR will review your submission to confirm that:

• The program will be used for a legitimate educational purpose.
• The requested program is appropriate for the stated educational setting.
• The intended use complies with ETR’s licensing and copyright requirements.

After reviewing your submission, ETR will take one of the following actions:

• Your educational use has been verified. ETR will provide purchasing instructions or proceed with the appropriate order and subscription process. 

•  ETR will contact you using the information provided on the form. You may be asked to clarify your educational setting, intended use, or requested number of users. 
•  ETR may be unable to approve the request if the proposed use is inconsistent with available license options or involves unauthorized reproduction, redistribution, resale, or public sharing of program materials. 

Important licensing information

ETR program materials are copyrighted and licensed for use by authorized individuals and organizations. Submission or approval of this form does not authorize you to:

• Share account credentials with unauthorized individuals
• Resell program materials
• Upload curriculum materials to public websites or shared repositories
• Distribute materials beyond the educators, students, locations, or organizations covered by the license
• Program materials may not be uploaded, entered, or shared with generative artificial intelligence (AI) tools or services. 
• Use an individual license for multiple unaffiliated educators or educational programs

Your order confirmation, subscription information, and applicable ETR terms will identify the use authorized under your license.
